The Shimano instructions call for one spacer on the drive side, but when I put it together like that on my IP-057 the chainline was horrible.  It worked better with no spacers, but I still get noisy engagement on the big chainring in front if I have it in the largest or second largest gear in the back.
Same here on my IP256SL. Don't use spacers, still the same noise like you descripted.

I thought it was 3 or 4 layers. With 1 or 2 you have to rub of the remainder.
For a good result, I needed 8 layers of red plasti dip.

Found an instruction video (pay attention from 1:20 and especially at 1:41) :

Got the same problem with my FSA headset (top cap).

This was what I did:
- Put the top cap half way in your fork.
- Tighten it a little bit and hold it on place by hand to prevent spinning
- Don't tighten to much, just a little bit.
- When it got enough resistance, press your top cap completely into you fork
- Now you're able to tighten your top cap.

I hope my explanation is clear enough ?

In this order: frame, bearing, lock ring, shims, top cap, optional spacers, stem, optional spacer, end cap.

I think you need some shims. I needed that also on my FSA headset (which I ordered from iPlay). If you top bearing drops in without resistance, and there's still room around the bearing, that's no problem. You need definitely some shims. With the shims the top cap pushes the bearing into your frame into a fix position.

I've got the same torque wrench and there is nothing wrong with this tools. Small, easy to use. Would buy it again. Maybe not a professional tool, but for occasionally use it's perfect.
